Surgical Management of Limitation of Motion due to Contracture of Gluteus Maximus

Abstract
Purpose: Contracture of the gluteus maximus is relatively rare, and its clinical features have previously been confused with herniations of lumbar disc disease, the piriformis syndrome of sequelae of cerebral palsy. This study presents the details and results of 9 cases of successful treatment of these contractures.

Materials and Methods: Nine patients, eight men and a woman, suffering from difficulty in squatting position or gait disturbance were included in this study. The mean age was 20.7 years old, and seven patients were bilateral. Four patients had a past medial history of multile intramuscular injection in the gluteal region. Preoperative mean measurements for further flexion, adduction and internal rotation of the hip joint were 42¡Æ, 15¡Æ, and -5¡Æ, respectively. After subsidence of acute pain, active and passive joint motion exercises commenced to prevent contracture recurrence.

Results: Postoperatively, the mean further flexion of the hip joint increased to 108¡Æ, and only slight rotation limitation remained. In follow up visits, no decreases in articular motion were observed.

Conclusion: The misdiagnosis of joint motion limitation due to contracture of gluteus maximus is common due to its clinnical features. So the meticulous exection/performance of differential diagnosis is essential. And careful surgical
management/stratgy accompanied with aggressive postoperative physical therapy is the most important focator in improving results.
